2025 Annuity Claim Forms
Instructions
1. There are several forms you must fill out, depending on what you are claiming. If you are claiming as an individual, fill out the individual form only. If you are claiming for a minor, fill out all relevant forms for the trustee claim forms along with an individual form for yourself.
2. If you are claiming for multiple minors, you must fill a form for each individual one.

3. All fields are required to fill out. If any fields are missing information, it may delay processing of claims

4. Send all completed forms to Annuity2025@whitefishadmin.ca. Do not send to any other email, as they may not be monitored and it could delay processing your claim.

5. Do not send your forms more than once. Doing so will slow processing of claims and does not make yours processed faster. We will notify claimants that we received the forms.

6. Below will be electronic options and printable options, if you wish to print and sign by hand.

7. If you wish to fill forms in person, you can pick them up at the band office starting Monday, December 15 2025
​
8. If you are claiming for a minor, you must provide all relevant legal documents proving guardianship. Failure to do so will delay processing.
Adobe Electronic Signature Instructions
If you are filling an electronic form but do not know how to add a signature, please follow the instructions provided by Adobe. Most phones and desktop computers have options to add signatures as well, but you may need to google them if you use another program.

These forms can be filled out on your computer web browser or phone. If you wish to fill in by hand, see the next section or receive a copy at the band office during work hours. Click on the link to automatically open/download your copy.

If you wish to simply print and sign a blank copy yourself, you can do so by clicking the link. The forms below do not have fillable spaces; they are for those who wish to do so by hand. Alternatively, you can receive a copy at the band office during business hours.
