If you are interested in registering for any of our services, please select the appropriate form and fill it with the required information.
- Please note that services are only for members residing on reserve unless otherwise stated.
- The forms are fillable online. If you wish to fill the form in person or need help with filling it, please visit the band office.
- Forms will only be reviewed for band members.
- Please only send one form at a time. Repeated form submissions will be ignored.
- Filled forms can be sent to the listed emails in the description of each department. Have the form title based on your name and what service you are applying for.
- There is no guarantee by the band office, or it's reviewing members such as chief and council or department heads, that the service will be provided even if the form is filled. If you have been accepted or approved, you will be notified.

Downloadable Forms
Some departments require clients to sign and register directly with them. If there are no forms for the department you are looking to register with, you may have to visit our offices during business hours to do so. Click on the title of each form to access and fill it.
If you are in need of housing on reserve, you can download and fill the following form. Please note that housing selections are up to the discretion of Chief and Council and the Housing Department. Whitefish Lake Band #459 does not guarantee needed housing will be supplied.

Post - secondary funding is available for those registered in college/university courses (non-upgrading). Funding is only available every new fall semester. The deadline for the new fall semester is May 15th, 2025. Please note that funding is only available for public post-secondary schooling. Private schools will not be considered. All filled forms should be sent to annao@whitefishadmin.ca.
If you are interested in becoming a member of Whitefish lake Band #459, please read through the attached form and verify if you have the information required to do so. Please send filled forms to glennan@whitefishadmin.ca
Social Assistance
If you are in need of financial social assistance, you will need to visit the main office, as this application requires an in-person interview. You can call into our main office at 780-767-3914. Please note that assistance is only available to on-reserve members.
