The forms that are needed to apply for financial support due to food insecurity caused by inflation can be found here. Please note the following:
There are two separate forms for individuals or claims for those with minors. If you are claiming a minor(s), you must complete the "WLFN #459 -2024 Food Insecurity Support Payment Application Parent/Guardian" available on this page. You must also complete the "Acknowledgement of Trustee, Release and Indemnity Agreement" form in order to legally claim that child and receive the funds for them. If you are not claiming a child/children, you must complete the "WLFN #459 - 2024 Food Insecurity Support Payment Application Individual" form instead.
2. This application is only available to band members.
For direct deposit, please include either a void cheque or direct deposit form from your bank. No other methods will be accepted
You must send all forms to 2024applications@whitefishadmin.ca DO NOT SEND TO THE WEBSITE EMAILS. IT WILL NOT ACCEPT THESE FORMS!
